A question asked in the right way often points to its own answer. Asking questions is the ABC of diagnosis. Only the inquiring mind solves problems. Great article, too many organisations hide from asking the simplest of questions and also hide from the answer they give….. I know a few who need to read this article. Very helpful, especially in the field of nursing where everything happen for so many reasons.
Someone has made millions explaining common sense. The 5-Why tool is a very useful tool but I find that if you are not experienced with facilitating a 5-Why session for Root Cause Analysis then it can lead you down the wrong path. Most problems do not have one cause but many interdependent causes. This tool looks at the many dependent and independent causes of an effect. Nice article. I have been practicing this and it definitely helps to understand the whys of the problem.
Also as part of it you first need to understand what is the problem and where the problem occurs. Is it at a particular time or during an acitvity , then when?
This way you can better understand the problem and get to the root cause of the problem. The example of the car running out of gas is actually a good example of how NOT to use the 5-Why method. Consider applying the logic used in this example to problems at your business. The easiest such demonstration of this is if we make the driver in this example an employee whose job it is to drive a company vehicle.
Teach your employee how to bluff? No, obviously not. The 5-Why was done incorrectly. The 5-Why should be used to reveal a failure of your process, not faults in employees. Why did the car stop? The process should be set up in a way to prevent it. I run into this kind of problem all the time while managing corrective action teams. The 5-Why is a great technique — fast, efficient, easy to understand — but it takes some thorough thought to keep it from going on weird tangents, and teaching this type of thinking is the challenging part of problem solving.
This is very useful when teaching laboratory management for future Medical Laboratory Scientists. Thank You! Interesting article. And this method has merit. Also, there are ten many reasons, or unclear reasons.
Eg why am I poor? Or because I chose the wrong market. Or because of BOTH of those. How does 5-questions handle this? The root causes identified in the two examples cited do not address the problems as they are not related to the problems. You write up on 5why analysis is very interesting. Can I know if there is any books I could buy to read this analysis. Trucker hooked to empty trailer traveled ft turned right then traveled more ft and trailer came in latched.
Driver checked latch with flashlight and says it was Latched. Why did trailer come unlatched? Nor do I think we got to the root cause of the issue with the first example — the root problem is not that the Dir needs to be in the loop — many problems were identified and need to be addressed even if the Dir did not need to be informed. Changing just the last whys do not fix all the issues in either example. Very well written and readable. Informative with good clear examples.
An example Ishikawa diagram would be helpful, but the article is plenty useful as-is. Thank you for writing and posting it. The example of 5 why regarding a car that stops because of a not good poker game player? I have not been able to find a bright line rule on when you reach the root cause.
Instead, I have developed some guidelines to help team decide when to stop asking why. A similar process of asking Why has been in use in the area of Psychological Development to discover less conscious values which drive our behavior Root Problem.
It is called the Values Unfolding Instrument by Dr. Because the Whys exercise is untethered from objective facts stats , the assumptions behind the questions and answers are critical. The answer to the 4th question is effectively that the person gambled all his money. He has a gambling issue. After the 5th Why question the conclusion is that he needs coaching in how to be better at gambling.
Should we give a person with a gambling issue training to get better at it? If this was done, the problem will happen again. You must be logged in to post a comment. Please Sign in Register. Let me explain. In most instances, it takes 16 weeks sometimes longer for Thorne to begin and finish the manufacturing of one of its nutritional supplement products. When it comes to your health and wellbeing, you should be absolutely certain that the supplements you take are of the highest quality, which is why Thorne spends so much time on testing.
Testing slows things down, no doubt, but Thorne believes product quality — which testing helps to achieve — should be top priority.
Thorne tries to account for raw material rejections, but on occasion, when a raw material fails that has no history of failing, or is replaced by a raw material that also fails testing, then we are left with no alternative but to search for a new, reliable source, which will extend the week manufacturing process and sometimes results in a product being on backorder for an extended period of time.
Recent examples of products that were on extended backorder because we had trouble sourcing a raw material that met our high standards are Crucera-SGS broccoli seed extract , Cortrex licorice root extract , Liver Cleanse dandelion extract , and Phytoprofen ginger extract.
Crafting and encapsulating high-quality supplements also takes time. Does it have to be? Again, it goes back to our higher standards. Some people could certainly say we make our supplements inefficiently.
For example, most supplement manufacturers mix magnesium stearate with raw materials during manufacturing. Other manufacturers use magnesium stearate or stearic acid because these substances lubricate machinery, helping ingredients to flow into the capsule faster and allowing more material to be packed in a capsule.
It means Thorne requires more time to encapsulate our products and more time to clean and sanitize our equipment. But it also means that when you take a Thorne supplement, you can trust that it is of the highest quality. The Thorne manufacturing team typically spends 12 hours cleaning manufacturing equipment, regardless of how long the actual manufacturing time took, which is usually hours.
Basically, that means Thorne spends much more time cleaning equipment and manufacturing areas than we actually do making our products.
